Service Request
We work predominantly with teenagers and adults experiencing mental health-related conditions​.
​
We use a two-step process for Service Requests and referrals. Our Service Request form captures some basic information about you (the Referrer) and the person requiring support services (the
Participant).
​
We initially ask for only the information that is needed to determine if we can help. This is to ensure we protect the privacy of Participants.
​
Note: If you are an NDIS Participant and you have a Service Co-ordinator, please ask your Service Co-ordinator to complete the Service Request for you and send a full copy of your Plan when we ask for it.
​
Once a Service Request has been accepted, we will ask for further details.​
